GLP‑1 medications / Tirzepatide

Tirzepatide (Zepbound®), prescribed and managed by your own physician.

A once-weekly injection that acts on two appetite hormones instead of one. Your physician decides whether it fits after reviewing your history and labs, and it always comes with a health coach and one-on-one visits.

SURMOUNT-1 trial result

20.9%

average body weight lost over 72 weeks on the 15 mg dose

Adults with obesity or overweight, without diabetes (Jastreboff et al., NEJM 2022). Averages, not a promise; everyone in the trial also made lifestyle changes.

What tirzepatide is

Tirzepatide acts on two gut hormones, GLP-1 and GIP, which together affect appetite, fullness and how the body handles sugar.

Zepbound®Dosed and FDA-approved for chronic weight management in adults with obesity, or overweight plus a weight-related condition, and for moderate to severe obstructive sleep apnea in adults with obesity.
Mounjaro®The same active ingredient, approved for type 2 diabetes. If you have diabetes, your physician will talk through which fits.
Compared with semaglutideIn the SURMOUNT-5 head-to-head trial, adults without diabetes lost an average of 20.2% of body weight on tirzepatide and 13.7% on semaglutide over 72 weeks. The better choice for you still depends on your history, side effects and cost.
Who it may not fitPersonal or family history of medullary thyroid cancer or MEN 2, pregnancy, or past pancreatitis. Your physician screens for these.

How dosing works

Zepbound starts at 2.5 mg and can step up by 2.5 mg every four weeks. Many people stay at 5 or 10 mg; 15 mg is the highest dose. Your physician may hold a step or go slower.

Darker bars are the maintenance doses (5, 10 or 15 mg). Once-weekly injection under the skin of the belly, thigh or upper arm. If you take birth control pills, ask about a backup method after starting and after each increase. Schedule from the Zepbound Prescribing Information.

Side effects and safety

The most common side effects are nausea, diarrhea, vomiting, constipation, stomach pain, indigestion and tiredness. They are usually worst in the first weeks after each dose increase.

Less common but serious risks include pancreatitis, gallbladder problems, kidney problems from dehydration, low blood sugar with some diabetes medicines, and serious allergic reactions. Call us, or 911 in an emergency, if you have severe stomach pain that won't go away.

Important safety information

Zepbound® carries a boxed warning about the risk of thyroid C-cell tumors. Do not use it if you or a family member has had medullary thyroid carcinoma or if you have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Not for use during pregnancy. Your physician reviews your history before prescribing.

Common questions

How is Zepbound different from Wegovy?

Zepbound (tirzepatide) acts on two hormones, GLP-1 and GIP; Wegovy (semaglutide) acts on GLP-1. In the SURMOUNT-5 trial, average weight loss over 72 weeks was 20.2% on tirzepatide and 13.7% on semaglutide. Side effects, your history and cost still decide which is right for you.

How is it given?

As a once-weekly injection under the skin, on the same day each week. We show you how at your first visit.

How effective is it?

In SURMOUNT-1, adults with obesity or overweight and without diabetes lost an average of 20.9% of body weight over 72 weeks on the 15 mg dose, compared with 3.1% on placebo. Individual results vary.

Does it affect birth control pills?

It can make birth control pills less reliable for a while. If you take them, use a backup or non-pill method for 4 weeks after starting and after each dose increase, as the Prescribing Information advises.

Do I have to follow a diet?

No strict diet and no calorie counting. Your coach helps you eat enough protein, stay hydrated and choose smaller portions that fit your real week.

Does insurance cover it?

Mindful Wellness is cash-pay and does not bill insurance for its programs. Program fees are FSA/HSA eligible, and we walk you through every cost up front, including medication.
